I'm not sure how I would go about changing the portraits of followers in game. I really dislike a few of the portraits and want to change em but i cant figure out how to go about it.

I do know how to add new portraits to the game for the players use, but it seems that to replace a portrait of a npc is much different.

Any help would mean a great deal.

All you have to do is replace a file in the portraits directory (in Arcanum3.dat). For example if you want a different portrait for Virgil, replace NPCVirg.bmp with the one you'd like. I haven't tried it yet, but I'm guessing the new picture should be 64 by 64 pixels since all the others are, too.
